For Your BenefitIf you had a wellness screening in 2013 as part of fulfilling the requirements for the Partnership Promise program, you may have recently received a letter from Onsite Health Diagnostics. The letter states that your personal information was included in a company data breach and that you are encouraged to enroll in free identity theft protection services by November 8, 2014.

The state of Tennessee announced this week that a data breach occurred involving personal information stored by Onsite Health Diagnostics, a subcontractor of the state’s wellness program, Onsite Health Diagnostics. The information that is believed to be part of the breach includes name, address, e-mail address, phone number, date of birth, and gender. No Social Security numbers, medical information, or employee IDs were compromised.
